Lotto Agent Requirements

REQUIREMENTS FOR A LOTTO MACHINE

(OBTAINING AN ELECTRONIC LOTTERY AGENT’S LICENCE)

All persons desirous of obtaining a “Lotto Agency” must complete an online application for an Online Electronic Lottery Agent’s Licence.

Should your application be approved, you will be required to provide (upload) the following documents:

 

REQUIREMENTS FOR APPLICATIONS BY INDIVIDUALS:

  1. Two (2) forms of National Identification (one copy of each)
  2. Utility Bill (not older than three months)
  3. Personal History Form (provided by the NLCB)
  4. Affidavit of Compliance (signed by Commissioner of Affidavit) (form provided by the NLCB)
  5. Police Certificate of Character (not older than six months)
  6. Bank Reference (not older than three months)
  7. Bank Statement (not older than three months)
  8. Deed / Lease / Proof of Tenancy & Permission Letter
  9. Tax Clearance Certificate

 

 

REQUIREMENTS FOR AN APPLICATION BY A COMPANY:

    1. Utility Bill (not older than three months)
    2. Personal History Form (provided by the NLCB)
    3. Affidavit of Compliance (signed by Commissioner of Affidavit) (form provided by the NLCB)
    4. Police Certificate of Character (not older than six months)
    5. Bank Reference (not older than three months)
    6. Bank Statement (not older than three months)
    7. Deed / Lease / Proof of Tenancy & Permission Letter
    8. Tax Clearance Certificate
    9. Vat Clearance Certificate
    10. Certificate of Incorporation
    11. Certificate of Continuance
    12. Current Annual Returns
    13. Notice of Directors
    14. Notice of Secretaries
    15. Notice of Address
    16. Company Seal

 

Incomplete applications will not be accepted.
